THE SPACE OF HYPERKÄHLER METRICS ON A 4-MANIFOLD WITH BOUNDARY

Let $X$ be a compact 4-manifold with boundary. We study the space of hyperkähler triples $\unicode[STIX]{x1D714}_{1},\unicode[STIX]{x1D714}_{2},\unicode[STIX]{x1D714}_{3}$ on $X$, modulo diffeomorphisms which are the identity on the boundary. We prove that this moduli space is a smooth infinite-dimensional manifold and describe the tangent space in terms of triples of closed anti-self-dual 2-forms. We also explore the corresponding boundary value problem: a hyperkähler triple restricts to a closed framing of the bundle of 2-forms on the boundary; we identify the infinitesimal deformations of this closed framing that can be filled in to hyperkähler deformations of the original triple. Finally we study explicit examples coming from gravitational instantons with isometric actions of $\text{SU}(2)$.

Objectives: The aim of this study was to develop a decision support tool to assess the potential benefits and costs of new healthcare interventions.
Methods: The Canadian Partnership Against Cancer (CPAC) commissioned the development of a Cancer Risk Management Model (CRMM)—a computer microsimulation model that simulates individual lives one at a time, from birth to death, taking account of Canadian demographic and labor force characteristics, risk factor exposures, and health histories. Information from all the simulated lives is combined to produce aggregate measures of health outcomes for the population or for particular subpopulations.
Results: The CRMM can project the population health and economic impacts of cancer control programs in Canada and the impacts of major risk factors, cancer prevention, and screening programs and new cancer treatments on population health and costs to the healthcare system. It estimates both the direct costs of medical care, as well as lost earnings and impacts on tax revenues. The lung and colorectal modules are available through the CPAC Web site (www.cancerview.ca/cancerrriskmanagement) to registered users where structured scenarios can be explored for their projected impacts. Advanced users will be able to specify new scenarios or change existing modules by varying input parameters or by accessing open source code. Model development is now being extended to cervical and breast cancers.

This paper considers developments in long-term care that are increasingly focused around the individual. Recent decades have seen massive changes in the way that care is understood and provided. Yet in Australia, as in Europe, North America and Asia, we are still a long way from a stable state of agreed services and provisions. Emphasising the social theory behind the shift, it is argued that understanding the individualisation of care cannot be reduced to a simple dichotomy of good or bad. Individualised care promises much, but the concept is applied to a wide range of phenomena, often in ways that conceal rather than reveal the character of the transactions involved. For individualisation to become meaningful it must be developed as a condition of recognition that is equally applicable to those who provide and those who depend on care. It is also important to distinguish individualised care finance arrangements from real attainments in the practice of providing care. These distinctions are necessary if we are to distinguish its use as an ideological justification for welfare cutbacks and the restructuring of care provisions as markets from the liberating potential that the approach can present when care practices are more truly based around the recognition of the individuals concerned: those who receive and depend on assistance as well as those who provide it.
